- A certified copy of your proof of identity such as your passport, driver’s license or Medicare card.
- A certified copy of proof of the relationship between the claimant and the policy holder such as marriage certificate or Medicare card.
- The original policy document and policy schedule for the insurance. If these can’t be found, a statutory declaration form will need to be provided.
- If the benefit is to be paid directly into a bank account, provide a direct credit authority.
